SERVICES: Physical Exams, Diabetes Management, Blood Pressure Control, Cancer Screenings, Nutrition Information, Limited Laboratory Screenings, Referrals for Vision Exams, Immunizations, Physicals, Lead Testing, Nutrition Information, Well Child Care, Limited Vision and Hearing Screenings, Developmental Checks, Dental Exams / Screenings, Gynecology Services, Clinical Breast Exams, Pap Screenings, Cancer Screenings, Nutrition Information, Pregnancy Testing, Referrals for Mammograms, Specialty Referrals

FEES: We are not a free clinic, but we do offer payment options based on a patient's ability to pay. Yes, we are a provider for Medicaid and Medicare plans as well as most private insurances. We accept Buckeye, Unison, and Care Source. A sliding fee scale is also available for the uninsured. It includes a required co-pay of $10.00 for Medical Services and $20.00 for Dental Services, which is due at the time of service
